Additional Info: Mr. “I have my own collet to prevent bit slips” AKA “Man who prides himself on not marking the spoilboard” has a large portion of crow to eat. The over-cut was definitely from the bit slipping; you can see the previous part of the same tool-path just barely marking the board. This looks like maybe 1/4” deep. 
But I will at least note that I stopped it after a single cut; I would’ve cut 10 more circles if I hadn’t been paying attention. And I am reporting the incident, as appropriate.
