Summary: Dust boot keeps falling off spindle
Additional Info: I added some double sided tape and tightened the screws, but it still fell off at least 3 different times on me. Fortunately, my toolpath was paused or feeding slowly, so I was able to stop the spindle each time before it ran over it.

The dust boot almost looks like it’s slightly oversized. Or maybe the metal is fatigued and stretched.

I saw blue painters underneath, over the collar and close to the (spinning) spindle nut. I removed most of this tape. I tightened the screw/nut on the left with a 4mm hex and pliers (for the nut). The dust boot has some movement but did not fall off. looks like the collar is tightened down all the way. A spot weld may have come loose or there may have been a shim that escaped. There was also blue painters tape over the brush door hinge which I left in place.
